December 30, 2007 With a heavy heart I have made the decision to close my practice effective March 1, 2008.  I will be joining Khan OB/GYN, inDover, on a full time basis.   All of my patients have the option of continuing their care with me at the new practice or choosing a new provider.  Please complete a release of information form so that your records may be transferred to Khan OB/GYN or to your new provider.  To pick up records after February 29, 2008 please fax the office at (866)-596-9890 or email the office at records@yourcnm.com. For patients needing to find another provider closer to your home, you may contact your insurance provider for a list of providers in your area.  Uninsured patients may contact the Screening for Life Program at 1-800-464-4357, the CHAPS program at 1-800-996-9969 ext 3 or Milford Public Health at 424-7140.
